Once you've completed this form, you must send in a CD (no tapes) for consideration.  Your CD will be added to the Battle of the FANS Intent To Play Library and considered in each submission panel.  If you're music changes, please submit new music and a new Intent to Play Submission Form.  This "Intent To Play Form" is in no way the rules &or regulations for future BOTF events.  If chosen to compete you will be required to sign a show specific "rules and regulations" before competing.
